Two Different Composite Markers Predict Severity and Threshold Dose in Peanut Allergy

چکیده
Background Safe and cost-effective biological surrogate markers to evaluate the severity threshold dose of peanut allergy (PA) reactions during an oral food challenge (OFC) are lacking. Objective To associated with allergic reaction OFC in a population children PA. Methods Demographic parameters basophil activation test (BAT) results were collected. Patients stratified into 2 groups (mild-to-moderate severe) cumulative groups: low (LCTG) ≤100 mg crushed high >100 mg. Results Among 68 included, there was 96% concordance between BAT result for diagnosis Of 56 positive (median age: 8.8 years), not correlated (P = .24). Higher Ara h 2–specific IgE FcεRI-positive control values both severe peanut. Combining these led 92% sensitivity (84%-97%) 82% specificity (71%-89%) all subjects. For LCTG, 4-variable composite marker, including age, normalized (EC50), FcεRI- fMLP-positive values, resulted 97% (89%-99%) 61% (49%-71%). Conclusion Distinct allergen-specific non–allergen-specific appear be

sIgE to peanut components does not accurately predict the severity of allergy in subjects suspected of peanut allergy
Methods Peanut allergic adults (n=94) and children (n=100) with a suspected peanut allergy were included. sIgE levels were measured by ImmunoCAP (Uppsala, Sweden). In all subjects the adapted Mueller score was used with one additional category for no symptoms (0-5): 0; no symptoms/peanut tolerant, 1; oral allergy symptoms, 2; cutaneous/mucosal symptoms, 3; gastrointestinal symptoms, 4; respirat...
متن کاملDiscriminant analyses of peanut allergy severity scores
Peanut allergy is one of the most prevalent food allergies. The possibility of a lethal accidental exposure and the persistence of the disease make it a public health problem. Evaluating the intensity of symptoms is accomplished with a double blind placebo controlled food challenge (DBPCFC), which scores the severity of reactions and measures the dose of peanut that elicits the first reaction. ...
متن کاملThreshold Dose Distribution in Walnut Allergy.
BACKGROUND In food allergy, eliciting doses (EDs) of foods on a population level can improve risk management and labeling strategies for the food industry and regulatory authorities. Previously, data available for walnut were unsuitable to determine EDs. OBJECTIVE The objective of this study was to determine EDs for walnut allergic adults and to compare with previously established threshold d...
متن کاملOral Peanut Challenge Identifies an Allergy but the Peanut Allergen Threshold Sensitivity Is Not Reproducible
BACKGROUND Double-blind placebo-controlled food challenge, DBPCFC, the gold standard for diagnosing food allergy, is time-consuming and potentially dangerous. A basophil allergen threshold sensitivity test, CD-sens, has shown promising results as a diagnostic tool in food allergy. OBJECTIVES To evaluate the reproducibility of oral peanut challenge and compare the outcome to CD-sens in peanut-...
متن کاملBasophil allergen threshold sensitivity, and peanut allergen components in relation to DBPCFC in children with suspected peanut allergy
Methods DBPCFC was performed with increasing concentrations of peanut allergen (1 mg to 5 g of peanut) in 42 children with suspected IgE-mediated peanut-allergy. Blood samples were taken for analyses of CD-sens and quantification of IgE-antibodies to peanut and Ara h 1-3, 8-9. Basophils were stimulated in vitro with peanut allergen in descending doses until the threshold sensitivity was reached...
